Hello, I am using Krita 4.4.1,
I encounter this little issue when I am assigning two new ‘available actions’ to ‘brushesAndStuff’ toolbar in krita, after restarting krita, one of the newly assigned action end up gone. The two new actions are ‘Reference image tool’ and ‘Clear’. When I restart, the reference image tool is always disappear.

perhaps you need a document open for it to appear?
not really though, the ‘clear’ action always appear even before opening a document but the ‘reference images tool’ is just not there. But it was still on the list of action when I open the ‘configure toolbars’ window.
I try adding another action which is ‘text tool’ a while ago to see if there’s any change but it resulting in the same thing like the first one, ‘text tool’ and ‘reference image tool’ are gone while ‘clear’ action is remain after restarting.
Since ‘text tool’ is displayed as a text instead of symbol (that capitalized T one) and ‘reference image tool’ is also displayed as a text instead of its symbol (the thumbtack one), perhaps that the toolbar won’t save any action without icons?
Yes. I have placed the “Reference Image Tool” and “Clear” and they work as expected, needs to have a document open to be usable but once I restart Krita the option (Reference Image Tool) on the menu is gone I need to place the option again for it to be active on the UI.
This looks like a bug to me and is easy to replicate.
Yes, thats exactly what I’ve been experiencing! 
And it’s also been bugging me (pun intended) how ‘Reference Image tool’ or ‘Text tool’ only appear as text instead of their usual icon/symbol like the one in toolbox
you should report those events to the bug tracker so they can fix it.
