Observe this gif, initially some pixels are painted and the tool behaves correctly, the reflection is produced exactly.
Then it is painted with a different brush and the reflection is no longer exact.
The reflection is located in the center of the canvas using the option of the tool itself “Move to center Canvas Y”
That is a known problem and @tiar has investigated it and raised a bug report: 427831 – Mirroring tool allows for subpixel placement without supporting it
It seems to vary depending on the brush used, as you’ve noticed.
